*Additional evaluation openings may be available, please contact us for all available appointments.

**Please note that a $60 deposit is required at the time of booking. This deposit will later be applied to any amount due following insurance coverage or refunded to your original form of payment. In the event of a no show or late cancellation (<24 hours' notice and not rescheduled) the fee is nonrefundable.
